The AAMC Community Health Center is designed to be a “primary care medical home” where a team of health professionals provides continuous, comprehensive, and coordinated care throughout a patient's lifetime. This team meets the needs of the uninsured and underinsured by providing access to affordable primary health care services, right here in our own community. The center is conveniently located on local bus routes.
Available services, by appointment, include:
• Primary care – preventive, acute, and chronic
• Consistent care through the patient's own physician or nurse practitioner
• After hours phone access
• Affordable health care
• Sliding fee scale, based on income
• Bilingual, Spanish-speaking staff
We serve individuals who have:
• Medical assistance
• Amerigroup
• Priority Partners
• United Healthcare Community and State (Americhoice)
• Primary Adult Care (PAC)
• The REACH Program
• Self-Pay (No insurance)
